1. Customer agrees
to inform Citi Lites Executive Transportation Service, Inc. (Hereafter
referred to as Citi Lites) within 24 hours of activity, any problem regarding
the service provided by Citi Lites including but not by way of limitation,
failure to provide the services in a timely manner, or any delay or dissatisfaction
in any service provided.
2. The customer agrees that if any dispute arises out of this agreement
or the service provided herein, the prevailing party shall be entitled
to recover all reasonable attorney's fees, interest and costs incurred.
3. Full payment shall be due upon receipt of all monthly statements.
4. The customer hereby agrees that the usual credit inquiries may be made
and it authorizes Citi Lites to obtain such information as it may require
from whatever sources it deems necessary concerning any statements made
on the application. In addition, the undersigned hereby authorizes Citi
Lites to charge account balances which are sixty days past due to pertinent
credit card(s) numbers of which are reflected on the reverse page.
5. The undersigned on behalf of the customer certifies that the above
statements are true, correct and complete and have been made by the undersigned
for the purpose of including Citi Lites to extend credit to the customer
knowing that Citi Lites will rely thereupon.
6. We are not responsible for personal property left in vehicles.
7. The undersigned represents that he/she is an officer and/or is duly
authorized to file the application for Citi Lites credit charge.
8. If payment is not received in 60 days, the undersigned agrees to allow
Citi Lites to charge the Credit Card listed on the accompanying Signature
on File Form.
